Regence Blue Cross Blue Shield of Oregon offers a wide variety of individual health plans along with individual dental options that can be added to any health insurance plan. The new plans are available 3-1-10. Current members will be converted to the plans below on 7-1-10.

Evolve Plus

• Deductible choices of $1000, $2500, $5000, $7500
• $25 copay for the first 4 doctor visits
• 80/20 coinsurance with a maximum of $4000 or $5500
• No deductible and 80% for Alternative care ($500 limit)
• Vision benefit of $150
• Rx deductible of $500 for brand name Rx

• $35 copay for the first 4 doctor visits
• 70/30 coinsurance with a maximum of $7500

Evolve HSA100

• Deductible of $5000 (individual) or $10000 (Family)
• 100% coverage after deductible
• Rx limit of $2000
• No brand name Rx coverage

• Deductible choices of $1500/$3500 individual, $3500/$7000 family
• 80/20 or 50/50 coinsurance option (maximum out of pocket: $5000/individual or $10000/family)

Stand Alone Dental

• Dollar based plan offers no deductible, but a six month wait period for any service
• Incentive based plan has a $50 annual deductible and no wait period
• Neither plan covers teeth bleeching, veneers or orthodontia
