Kaiser Permanente is solely an HMO. They use the Providence network if services cannot be treated at their facilities. Premiums are typically higher than PPO plans, however, your out-of-pocket expenses are much lower. Kaiser is the most convienent plan for consumers. You can elect not to have Rx coverage to help save premium.

Platinum

• No deductible. Maximum out of pocket of $2500

• Maximum out of pocket of $4500 plus deductible
• $25 copay for primary care provider
• 70/30 coinsurance after deductible

• Deductible choices of $5000 or $7500
